- Function as a Program Coordinator for the individual(s) served.
- Assist in the development, preparation, and monitoring of individual(s) served ISP according to the guidelines of DMRS and other entities that provide regulatory oversight and/or funding, and according to the needs of the individual(s) served.
- Interview prospective individual(s) served and families.
- Coordinate and arrange appointments, meetings, etc.
- Serve as a support for the individual(s) served, the individual(s) served's family, and Evergreen staff who provide services.
- Monitor budgetary requirements and expenditures.
- Ensure that all policies, procedures, regulations, and guidelines are followed.
- Assist with scheduling individual(s) served for specific training and maintaining documentation of such training.
- Will be on call and assist the Program Supervisor and Executive Director as needed in performing any other tasks as assigned.
- Abides by all policies, procedures, regulations, and guidelines for safety, quality assurance, and general corporate oversight.
- Meets with the individual(s) served, families, persons from other agencies, including state agencies, medical and other service providers, to develop individualized programs and services for the individual(s) served.
- Coordinates services, monitors, and revises programs, and generally provides oversight of individual(s) served health and welfare.
- Employs and trains staff and monitors their work performance.
- Performs all supervisory duties as required.
- Reviews written and oral reports on matters relating to the individual(s) served and acts through verbal and/or written instructions on these matters.
- Assist in the performance of other duties required by the needs of the individual(s) served and as assigned by the Executive Director or Program Supervisor.
- Acting Strategically: Identifies key issues and relationships relevant to achieving goals and commits to a course of action to accomplish goals based on known and unknown variables. Develops priorities with the right balance of short-term and long-term considerations.
- Championing Customer Needs: provides timely and professional service to both internal and external customers, is responsive to customer needs and requests, is always courteous to the customer, and considers them when making decisions.
- Communicating Effectively: Share information. Listens and involves others. Conveys ideas in a manner that engages others and helps them understand and retain the message.
- Delivering High-Quality Work: Makes sure responsibilities central to the role meet all requirements and expectations. Finishes tasks promptly and critically reviews work for accuracy and quality. Consider the impact of work on others.
- Prioritizing and Organizing Work: Allocates time and attention based on what is most important to achieve key goals and objectives. Effectively organizes and balances tasks and priorities to keep multiple projects on track.
- High school diploma or equivalent.
- Bachelor’s degree in a related Human Service field preferred.
- Must know how to determine the needs of individuals served and develop programs that will meet those needs.
- Ability to calculate wages to assist individuals served
- Prior work in a non-profit atmosphere and experience with people with intellectual and developmental disabilities are preferred.
- A combination of education and experience will be considered.
- Occasionally exposed to viruses and infectious conditions.
- Must have good physical and mental health and be able to work in a fast-paced work setting.
- Must be able to deal with stress and adjust to a flexible work schedule.
- Must be able to travel to various areas of the state and out of state, as well as work unscheduled shifts when the need arises.
- Constantly moves about to coordinate work.
- Regularly works in a fast-paced environment with multiple task deadlines.
- Regularly moves and positions objects weighing up to 50 pounds.
- Occasionally exposed to viruses and infectious conditions.
- Constantly alert and observant during working hours.
- Will supervise.
- Must have understanding, patience, and tact in dealing with the individual(s) served and their families.
- Must be creative in problem-solving and planning.
- Must have the ability to maintain good working relationships with staff, professionals, and persons from other agencies.
- Working knowledge of Microsoft Windows and Office applications.
- Working knowledge of Google Workspace.
- Aptitude to learn other software programs as required for this position.
- Must have a good driving record that meets the minimum requirements for Evergreen and reliable transportation.
- Must pass a drug screen and criminal background check.
- Will frequently have direct contact with the individual(s) served and their families.
- Availability for evening and weekend work may be required based on operational needs.
- May be required to work in a variety of settings and environments, both indoors and outdoors.
